What To Prepare For Your Appraisal

 

You will have your appraisal through the MAG (Medical Appraisal Guide) Form. A PDF which includes all the requirments of the appraisal where you attach documents/supporting documents (ie. feedback forms, signed letters confirming CPDs/or of teaching done). Your can download the MAG Appraisal form below (you may have to download the latest Adobe Reader and then open the downloaded MAG form directly from Adobe).

1. Personal development plan - (usually you have set yourself one at your previous appraisal/ARCP) and you set a PDP for the following year.


2. CPD (Continued professional development) Points. These can be anything outside of your normal work which you can count as developing your professional career. Normally 1 hour = CPD point. Examples - Attending grand rounds, departmental teaching, journal clubs, MDT meetings, online modules, any courses, teaching, anything else you can prove you have done that you think should count as a CPD. GMC actually state they do not require a doctor to have a specific number of points, but as a guide we would suggest attempt to show evidence for around 50 points for the year if possible.

3. Colleague and Patient Feedback - Questionaire forms you can give out to which colleagues and patients of your choice. You can collect these at any time during the year. You can download the GMC questionaires below.

How Your Appraisal Is Scored

 

Your appraisal will be split into the following categories and to pass your appraisal you must contain sufficient information in these areas.

 

  • Scope of Practice

  • Outcomes

  • Complaints and incidents

  • CPD done in the last year and reflection on how it improved your clinical skills

  • Quality Improvement Activity

  • Colleague and Patient Feedback

  • Patient Safety

  • Probity and Health Issues

  • Summary

Score 2 if there is sufficient information to answer the question

Score 1 if mentioned but unclear

Score 0 if not mentioned

1. Scope of practice: Possible score = 6

  • Nature of practice and subspecialty interest - score 2

  • Numbers seen/procedures performed (workload) - score 2

  • Work in other hospitals, private practice. If none, say so - score 2

2. Outcomes: Possible score = 2

  • Reference to outcomes - score 1

  • Measures used – score 1

 

3. Complaints and incidents: Possible score = 2

  • If none, say so – score 2

  • If referred to – score 1

  • If remedial action described – score 1

NB, if you have not been cited personally, include examples relating to your team.

4. CPD and Quality improvement activity: Possible score = 3

  • Description of nature of activities – score 1

  • How it will improve practice – score 1

  • Does this tie in with proposed PDP – score 1

 

5. Colleague and patient feedback: Possible score = 2

  • Reference to colleague feedback – score 1

  • Reference to patient feedback – score 1

 

6. Patient safety: Possible score = 2

  • Clear reference to how you ensure patient safety in general, and in your area of expertise – score 2 

(Any reference to above – score 1)

 

7. Probity and health issues: Possible score = 2

  • Any reference to probity – score 1

  • Any reference to health issues – score 1

 

8. Summary: Possible score = 2

  • Make a clear statement about scope and quality of practice and any changes that might occur in next year – score 1

  • Make a clear statement about any concerns and how they will be managed. If none, say so – score 1

 

Total score possible = 21
